About Breast Augmentation Surgery
Breast augmentation, also known as augmentation mammoplasty or breast implants, is a surgical procedure to enhance a woman's breast size and breast shape. Breast augmentation is sought out at the Body Sculpting Center in Scottsdale, Arizona for a number of reasons; to enhance the body contour of a woman who, for personal reasons, feels her breast size is too small, to correct a reduction in breast volume after pregnancy, to balance a difference in breast size or as a reconstructive technique following breast surgery. By inserting a breast implant behind each natural breast, cosmetic and plastic surgeons are able to increase a woman's bust line by one or more bra cup sizes. If you are considering breast augmentation, this will give you a basic understanding of the procedure and help you answer the following questions:

Breast augmentation can enhance your appearance and your self-confidence but a breast augmentation will not necessarily change your looks to match your ideal, or cause other people to treat you differently. Before you decide to have breast augmentation surgery, think carefully about your expectations and discuss them with your surgeon. The best candidates for breast augmentation are women who are looking for improvement, not perfection, in the way their breasts look. If you are physically healthy and realistic in your expectations, you may be a good candidate for breast augmentation surgery. Breast augmentation surgery can have a positive effect on a persons self-image when performed for the right reasons. The most common reasons for having breast augmentation surgery are:

  • Add fullness to the breasts creating a more proportional body contour
  • Reshape breasts that have lost their fullness after childbirth and feeding
  • Give symmetry to uneven breasts
  • Reconstructing breasts after mastectomy

Due to potential risks, women who are currently pregnant or nursing, have a malignant or pre-malignant cancer in their breast(s) and women who have an active infection anywhere in their body are not eligible for breast augmentation surgery.

Surgical Methods For Breast Augmentation
The four most common approaches taken by a surgeon performing a breast augmentation surgery are: transaxillary, periareolar, inframammary and transumbilical. These names refer to the location of the incision for the breast implant during surgery (see illustration below).

Each of these approaches to breast augmentation have their own advantages and disadvantages. Patients receiving breast augmentation at the Body Sculpting Center in Scottsdale, Arizona, using either the transaxillary or periareolar approach can expect to resume most normal activities within 24 hours of surgery. The breast implant (saline or silicone) may be placed either above the pectoral muscle (subglandular), below the pectoral muscle (subpectoral), or (subfacial) between the muscle and its covering (fascia), depending on the amount of existing breast tissue and the patients desired outcome (see illustration below).

Transaxillary Breast Augmentation
The transaxillary approach to breast augmentation refers to the incision for the breast implant being hidden in the skin folds of the armpit. There is no actual incision on the breasts themselves. The scar left by the transaxillary breast augmentation approach is hardly, if at all, visible even when the arms are held straight up over the head. Periareolar Breast Augmentation
The periareolar incision for breast implants is camouflaged by hiding the incision in the border of the areola (the sensitive pigmented skin surrounding the nipple) where the darker pigmented skin meets the lighter pigmented skin. Any scarring can be further reduced by using a zigzag incision as opposed to a straight incision. Inframammary Breast Augmentation
The inframammary breast augmentation approach is by far the most widely used and reliable method for placing a breast implant. The incision is placed below the breast on the crease of the breast. The inframammary incision method makes for easier re-augmentations and revisions as well. One drawback to using the inframammary breast augmentation method is that the scarring can be visible to some extent, mainly when the arms are held straight over the head or during intimate contact. A patient who objects to a slightly visible scar should consider one of the other breast augmentation approaches. Transumbilical Breast Augmentation (TUBA)
The transumbilical approach to breast augmentation is popular because it allows both breast implants to be placed with the assistance of an endoscope through a single incision in the upper fold of the belly button with no visible scarring. The transumbilical breast augmentation method is fairly new, having been developed in the early nineties, but has steadily gained in popularity. Types of Breast Implants
A breast implant is a silicone shell filled with either silicone gel or a salt-water solution known as saline. Silicone implants were banned by the FDA in 1992, they are currently pending FDA approval due to new studies and reconsideration of former allegations. Saline implants are the only breast implant currently approved by the FDA.

The breast implants come in different shapes as well, moderate, moderate plus and high profile. Moderate breast implants have been used for over 30 years, and offer a full round look that adds greater dimension to the chest. The high profile breast implants have become increasingly popular in breast augmentations because they provide a greater amount of projection to the breasts on a narrower chest. About 24 Hour Recovery From Breast Augmentation Surgery:

For the past three decades breast augmentation surgery has been performed using procedures that not only tear the tissue, but also bruise the bone and create excessive bleeding, making it very uncomfortable for the patient after surgery. In order to treat post-surgery pain the patients were given narcotics and instructed to reduce activity for several days. This promotes cramping and stiffness of the muscles and prolongs the healing process.

Dr. Borsand and the staff at the Body Sculpting Center in Scottsdale, Arizona have adopted new surgical and post-operative techniques, implemented by Dr. John Tebbetts of Dallas (author of 'The Best Breast'), TX. These new techniques usually eliminate what is known as “blunt dissection” or tearing of the tissue and instead use a method known as electrocautery forceps dissection. During surgery, Dr. Borsand now ensures that the sheath that provides sensation to the bones, known as the periosteum, is not touched, minimizing bruising and significantly reducing post-operative discomfort. In addition, the new techniques provide a surgical environment with a goal of zero bleeding, further reducing post-operative discomfort since blood acts as an irritant and causes inflammation of the tissue. Due to the special care taken by Dr. Borsand during surgery, post-surgical pain is reduced so drastically that any discomfort the patient feels can be treated with a mild analgesic. After surgery, each patient is required to follow a strict guideline of activities. They will begin arm exercises immediately after surgery and perform 5 sets every hour after surgery while awake for 2-3 days. The patient is encouraged to shower and go out to dinner within just a few hours of leaving the facility. Patients who take an active role in their recovery are integral to the success of the 24-hour recovery process.

Patients undergoing inframammary (incision in the fold below the breast) or periareolar (incision hidden in pigment of the areola) breast augmentation are good candidates for the 24-hour recovery since the implant is placed directly through incisions in the breast and give Dr. Borsand a direct access to the implant pocket. Other methods such as transumbilical (through the belly button) and transaxillary (through the armpit) are not good candidates because direct access is not as readily available.

Standard 24-Hour Breast Augmentation Recovery :

ACTIVITY:
You will take a short (2 hour) nap after surgery and then resume normal non-strenuous activities. You may raise your arms above shoulders as needed. No vigorous activity for at least the first three weeks. When you do resume this activity, you should always wear a sports bra. You may drive a car and do light housework 24 hours after your general anesthesia if you feel up to it.

POSITION:
Keep yourself propped up on a couple of pillows while resting to help reduce swelling. You may sleep in any comfortable position.  

POST-OPERATIVE VISITS:
You will visit our office 1-4 days after the surgery to be evaluated and receive a breast stabilizer if your implants are under the muscle. You will receive additional instructions at that time regarding post operative care. All your sutures are under the skin and no suture removal is required. You may have steristrips or specialized tape on your incisions.  Leave this closure alone until instructed to remove

DRESSINGS:
The bra and ace wrap serve as the dressing to prevent initial bleeding.Try to keep the ace wrap over the breast like a tube top until instructed to remove it. We want you to wear a bra at all times for 1 week. After 2 weeks, you may sleep without the bra. You should not go braless during the day for 2 weeks after surgery. No under wire bras for three months.

 
SHOWERING AND BATHING:
You will be instructed when to remove the wraps and shower by the recovery room staff.

VITAMINS:
Take vitamin E (400 units) twice a day for 6-8 weeks, starting after 7 days.  

DEODORANT:
Patients receiving the breast implant incision in the arm pit should not wear deodorant for 1 week following surgery in order to avoid infection. 

EXPOSURE TO SUNLIGHT:
Scars take at least one year to fade completely. During this time, you must protect them from the sun. Even through a bathing suit, a good deal of sunlight can reach the skin and cause damage. Wear a sunscreen with a sun protection factor (SPF) of at least 15 at all times when in the sunshine. Be extremely careful if areas of your breast skin have reduced sensitivity. No tanning nude breasts for 1 year, and no sunbathing or tanning for 2 weeks.

SWELLING:
The first 4 weeks your breasts will sit up high and feel hard after which the breast implants will slowly start to settle. In general, swelling lasts for a total of 6-8 weeks.
 
LIFTING:
No lifting anything over 5 pounds for the first week, 10 pounds the second and 15 pounds for the thirrd week.

EXERCISE:
You may take gentle walks the day of surgery, go out to dinner, walk around the mall etc. Do not return to aerobic exercise for 2 weeks. Starting the day of surgery lay on your chest  every night for 15 minutes until it is no longer uncomfortable to do so. After your first appointment you will be instructed to perform "nipple placement" exercise by sliding hand to bottom of breast and pulling up the skin. This exercise brings the nipple back into correct position. Perform breast "massages" for 2 months after surgery by taking right fist and placing it on left nipple. Take left hand, place on fist, and push in. Hold for 15 seconds. Repeat on other breast with opposite hands, and perform the exercise 5-6 times each day.

 
SPECIFIC “AS YOU HEAL" INFORMATION
 
ASYMMETRY:
It is quite common for the two breasts to heal differently. One may swell more, one may feel more uncomfortable, or the shapes may differ initially. After complete healing, they should look remarkably similar and natural. Patience is required, but if you are concerned, ask questions of your surgeon or the nursing staff.
 
ACITIVITIES:
No aerobics, running, or jumping sports for 2 weeks. When you do start these activities, you should always wear your sports bra. You need to consult your surgeon before starting any daily workouts.
 
HEALING OF SENSORY NERVES:
Regeneration of the sensory nerves is also accompanied by tingling, burning, or shooting pains, which will disappear with time and are nothing to be alarmed about. You may exert pressure with the palm of your hand over the area until the pain is gone.
 
SENSITIVITY:
As the nerves regenerate, the nipples commonly become hypersensitive. This will subside with time. You may find that gentle massaging helps.
 
"SLOSHING" SENSATION:
You may hear and feel "sloshing" in your breast after surgery. This is not the implant! It is the air that is trapped in the space around the implant and the natural fluid that accumulates after an operation. It will all be absorbed by the body within a few weeks.
 
SHINY SKIN:
The skin of your breasts may become shiny. This is a result of the swelling that occurs after surgery. Within a few weeks, the edema and swelling will subside, and the skin will look more normal.

COMMON BREAST AUGMENTATION RISKS & COMPLICATIONS:
The most common complication resulting from breast augmentation surgery is called capsular contracture. When a breast implant, or any foreign object, is placed under the skin scar tissue begins to develop around it creating a capsule. In some women, the capsule can tighten and squeeze the breast implant creating a hard shell. If caught in the early stages, capsular contracture can be corrected by the doctor with a simple non-invasive method of squeezing the breast tissue. If capsular contracture worsens, a surgical procedure is required to remove the encapsulating scar tissue from the breast.

To help prevent capsular contracture before it happens, the patient is given exercises to perform after her breast augmentation surgery that reduce the risk of capsular contracture. In the event of capsular contracture, the patient will notice that the breast has hardened and is no longer malleable or that one breast is considerably less malleable than the other.

Another common complication is deflation or rupturing of the implant. Breast implants deflate when the saline solution leaks either through an unsealed or damaged valve, or through a break in the breast implant shell. Breast implant deflation can occur immediately or progressively over a period of days and is noticed by loss of size or shape of the breast implant. Some breast implants deflate (or rupture) in the first few months after being implanted and some breast implants deflate after several years. You should also be aware that the breast implant may wear out over time and deflate/rupture. Deflated breast implants necessitate additional surgery to remove and to possibly replace the breast implant. The body safely absorbs and excretes the saline solution leaked by a deflating or ruptured implant.
 
The dangers of breast cancer are no greater after breast augmentation surgery, and self examination of your breast post-operatively is still possible and important.
