Phoenix Phalloplasty Procedures
Phoenix Phalloplasty: Penile Lengthening at Body Sculpting Center in Phoenix, Arizona
Penile Lengthening is a relatively new technique in the plastic/cosmetic surgery field. It was first performed in China, in 1984, by Dr. Long Daochau. Dr. Daochau performed the surgery on a man who, as an infant, had his penis bitten off by a dog. The success of this procedure made it possible to offer it to adults who have a normal size penis, but who are unhappy with the length of their penis.
In the relatively safe environment of the doctor’s office, many men confide that they experience embarrassment and lowered self-confidence when in the locker room with other men or when in the bedroom with women. They report that they feel “less a man” because of the size of their penis. They believe that having a longer penis will improve their self-esteem and self-confidence, and decrease their constant frustration with the size of their penis. Before surgery, patients usually express excitement. After the surgery, most of them speak enthusiastically about their experience, express satisfaction with the results, and often report a positive sense of feeling "sexier". Of course, there are some patients who will never be satisfied with the surgeon and/or the surgery, no matter how surgically perfect the operation.
Scottsdale phalloplasty surgery primarily increases the non-erect (flaccid) length of the penis and, to a lesser extent, the erect length. The average penis length that a patient gains as a result of this surgery varies from person to person, and may or may not fall into the normal range of one-half to one-and-one-half additional inches in the flaccid position. It is not possible to determine before surgery how much length the patient will gain from the surgery. Separation of the penis from the pubic bone, combined with suprapubic liposuction, enhances the length of the penis, and additional length can be gained by repositioning the scrotal pubic fold closer to the pubic area.

The girth of the penis refers to the circumference of the penis. Girth enhancement refers to a thickening of the girth of the penis, or an increase in penile circumference. In order to increase the girth of the penis, something must be added to it. A previous technique that used fat transfer (Free Fat Transfer) had many disadvantages, including lack of uniform fat distribution, secondary to uneven fat absorption; shrinkage, secondary to constant fat absorption and disappearance over time; and lumps, clumps and bumps on the shaft of the penis, secondary to fat sequestration and fat graft displacement during sexual activities. The best solution is a graft that can be attached in a flat position and stitched to the penile fascia to prevent it from moving during sexual activities.
Currently, there are two types of permanent graft available to increase the thickness (girth) of the penis. The first type of graft uses the patient’s own skin (dermal fat graft or DFG). Girth enhancement with dermal fat grafts (DFGs) requires peeling the epidermis from the harvested skin graft before placing it on the shaft of the penis. The second type of graft is AlloDerm®.
Girth enhancement surgery is performed under general anesthesia. The surgery requires two independent incisions: one at the base of the penis, and one on the neck of the penis, just below the corona. A graft is then placed in the appropriate area on the shaft of the penis. This area is usually the anterior three-fourths of the penile shaft.
Girth enhancement surgery can increase the circumference of the shaft of the penis from a 10% increase to a 30% increase over the existing pre-operative circumference size, depending on the number and size of the grafts used during this surgery. This maximum degree of enlargement is not possible for everyone, due to anatomical differences between individuals. The patient must have realistic expectations when undergoing this surgery.

The glans of the penis is the rounded, gland-shaped head of the penis. It is composed of a spongy structure that is the expanded cap of the corpus spongiosum (the spongy cylinder that surrounds the urethra). The glans surrounds, and covers, the rounded ends of the corpora cavernosa, two long, cylindrical, erectile bodies that parallel the corpus spongiosum within the penis. At the bottom, or border, of the glans there is a rounded structure called the corona, and just below this corona is the neck of the penis.
The size of the glans, in proportion to the size of the penis, can vary greatly, even in the flaccid state. In the erect position, any increase in the size of the glans depends upon the amount of blood that this spongy structure can contain.
During erection, the amount of blood inside the corpus spongiosum increases. While inflow increases, outflow decreases, and the corpus spongiosum is filled to the limit with blood. As a result, it stands upright, causing erection of the glans of the penis. This process occurs simultaneously with erection of the penis as a whole, caused by a parallel intake of blood in the corpora cavernosa. An enormous nerve network inside the glans makes it the most sensitive part of the penis.
Glanular enhancement techniques have been developed and successfully implemented in surgical penile augmentation practice for some time. There are two variants of glanular enhancement: temporary and permanent. Temporary glanular enhancement is an augmentation that requires maintenance to preserve the desired enlargement. Permanent glanular enhancement is an enlargement technique that uses permanent grafts, such as a graft of the patient’s own skin (dermal fat graft, or DFG) or AlloDerm (R).
