Silhouette® Mid-Face Lift

This spotlight is on a new medical device for mid-face lifts called the Silhouette® suture. The Body Sculpting Center’s Dr. Leon has been using these sutures and achieving great results.

Dr. Leon at the Body Sculpting Center has been performing mid-face lifts using a new medical device. Some of you may remember us introducing a barbed suture to our practice a year or two back that is inserted into the superficial subcutaneous tissue of the face or other body parts and is used to lift and hold the skin in a more firm position.

Fairly recently, a company named KMI made some modifications to the original barbed suture design that allows it to hold the tissue with longer-lasting results called the Silhouette® suture. The barbed sutures used before were made by making cuts in the shaft of a normal suture, therefore weakening the structure. KMI has incorporated small hollow cones throughout the Silhouette® suture’s length, allowing them to anchor themselves better without weakening the structure (see illustration).

What does this mean to the patient? Well, simply put, the patient will have just as great of a result as before, if not better, and the result will last quite a bit longer. Take a look at the before and after photos. Notice the difference in the jowels as well as the firmness of the rest of the lower and mid-face. This procedure is ideal for almost anyone who is showing moderate laxity in the skin due to aging. Insertion

The Silhouette® mid-face suture is inserted through a small incision in the temporal area using the attached needle. The suture goes through the superficial subcutaneous tissue exiting at the desired location in the malar or submalar perioral area.

Deployment

The Silhouette® mid-face sutures are deployed in the superficial libro-adipose tissue of the face (lower temporal, malar and perioral area). The sutures are placed slightly parallel to each other, but with a broader base at the level of the lower face and decreasing separation as the temporal area is approached.

Anchoring & Suspension

The Silhouette® mid-face sutures are pulled backwards through the temporal area until the cones engage the fibroreticular fibers of the subcutaneous tissue of the face. Further traction with the Silhouette® sutures will produce mobilization and displacement of the facial tissues, suspending them to a higher desired location.

Fixation

The Silhouette® mid-face sutures are fixed in pairs to the deep temporal facia which has been reinforced with a non-absorbable mesh at the level of the temporal incision using a preloaded half-curved non-cutting needle.

G-Spot Enhancement

The G-Spot enhancement procedure is one of the newest procedures available for the female body and is intended for women who would like to increase their ability to achieve orgasm through vaginal stimulation.

The G-Spot is more formally known as the Grafenberg Spot and is named after the Berlin-based gynecologist, Ernst Gräfenberg, who first published an article in the International Journal of Sexology on the subject in 1950 entitled “The Role of Urethra in Female Orgasm.” The G-spot is a lima-bean shaped spongy area surrounding the urethra. It contains clusters of nerve endings comparative to the male prostate. It is located up to three inches inside the vagina on the front (anterior) wall (side view diagram). The G-Spot location varies in women somewhere between 11 o'clock and 1 o'clock (front view diagram) when viewed from the front. When stimulated, the G-Spot can produce an intense orgasm and in some women may cause a thicker ejaculation. G-Spot stimulation, which takes place inside the vagina, is not to be confused with clitoral stimulation, which takes place on the outside of the vagina. The G-Spot, in its hard-to-reach location, often requires direct pressure to generate an orgasm. This is usually from fingers, although in some positions may be stimulated by the penis.

At the Body Sculpting Center in Scottsdale, we offer the G-Spot enhancement procedure that uses an injection of the cosmetic filler known as Restylane®. The Restylane® injection enlarges the G-Spot, making it easier to stimulate and achieve G-Spot orgasm. The effects last for approximately 3 to 6 months. Other cosmetic fillers may be used for G-Spot enhancement, but our studies have shown the best success with Restylane®.

FDA Says "Yes"
Two New FDA Approvals

There are two recent FDA approvals that we would like to point out in this issue. The first one is a long awaited approval for silicone breast implants. For the past 14 years, silicone-filled breast implants have been off the market and only saline implants have been available. Implant manufacturers such as Mentor® and Inamed® have been working hard to develop new silicone breast implants that meet the specifications given to them by the FDA. At this point, Mentor® has been approved by the FDA for the use of their MemoryGel™ silicone breast implants for breast augmentation in patients 22 years of age or older and for breast reconstruction on patients of any age. Surgeons and patients have been pushing for the return of the silicone implant to the market due to its more natural feel.

The second FDA approval that we would like to touch on in this issue is for Radiesse®, an injectable dermal filler formerly known as Radiance® that has had some success on the market prior to FDA approval. Radiesse® is commonly used for filling in fine lines and wrinkles around the nose and mouth including nasolabial folds, marionette lines and smile lines. Radiesse® has also been used for other treatments such as lip enhancement. Radiesse® is made up of tiny calcium-based microspheres to support and stimulate the growth of the body’s own collagen. Radiesse® has proven to have longer lasting results than other dermal fillers on the market such as Restylane® and CosmoPlast®.

New Saline Lip Implant

Another new product for cosmetic surgery has hit the market. This one is a lip implant, called Fulfil®, that shares many similarities to a breast implant. The Fulfil® implant, which is a small cylindrical bag originally intended for use as a nasal splint, is threaded into the subcutaneous tissue of the lip on a long suture. Once it is aligned inside the lip, a syringe is connected to the fill tube on the implant and the implant is inflated to the desired size. The fill tube is then removed, and the implant self seals. The remaining sutures are clipped and the lip is ready to go. It is a quick procedure with instant results and almost no down-time for the patient. The procedure can be performed under a local anesthetic and takes approximately 10-15 minutes per lip.
